How FST Care Club Works
1. Choose Your Plan
Select the plan that best fits your needs: Adult, Perio, or Child. If you have multiple family members, you can enroll them in different plans as appropriate.
2. Enroll and Activate
Complete a simple enrollment process and your membership is immediately activated. You can start using your benefits right away.
3. Schedule Your Visits
Call to schedule your preventive care appointments. Routine cleanings and exams are covered by your membership with no additional out-of-pocket costs.

Is there a contract or commitment period?
FST Care Club is an ongoing monthly membership with no long-term contract, and you may cancel at any time by calling our office. Please note that dues already paid are non-refundable, and memberships are non-transferable to another dental office.
What happens if I miss a monthly payment?
Please contact us right away so we can help keep your membership active. Membership dues must be current in order to receive member services, and if dues lapse for more than 28 days a reactivation fee applies to restart the membership.
Can I use my membership immediately?
Yes! Your membership is active immediately upon enrollment. You can schedule your first appointment right away.
Are emergency visits covered?
Your plan includes a problem-focused exam and the necessary diagnostic x-ray when you come in with dental pain, so we can evaluate the issue promptly. Any treatment that follows, such as a filling, extraction, or root canal, is billed separately at your member-exclusive discount.
Can I add family members?
Absolutely! Family members can enroll in FST Care Club at any time. Each person selects their own plan based on their needs.
What if I need treatment not covered by my plan?
When you need restorative or cosmetic treatment, you'll receive exclusive member discounts on those services. We'll discuss options and pricing before treatment begins.
Is FST Care Club insurance?
No, FST Care Club is a membership program, not dental insurance. It provides affordable preventive care coverage without the hassles of insurance claims and pre-authorizations.
How often can I have cleanings?
Most patients benefit from cleanings and checkups every six months. Some patients with gum disease or high cavity risk may benefit from more frequent visits, which we can arrange.
Membership Terms & Disclosures
This Program Is Not Dental Insurance
FST Care Club is a discount membership program offered directly by Jasleen Kaur, D.M.D., Inc., doing business as Forever Smiles Turlock. It is not dental insurance, not a health plan, not a dental service plan, and not a plan of health care service regulated under the Knox-Keene Health Care Service Plan Act. No third party pays for your care. There is no insurer, no claims process, no deductible, no annual maximum, and no coordination of benefits. Your agreement is directly with this dental practice.
Who Is Eligible
Membership is available exclusively to patients of our practice who do not carry dental insurance. If you obtain dental insurance while enrolled, please notify our office so we can bill your plan appropriately. Membership may not be combined with dental insurance for the same services.
What Your Membership Includes
Membership covers only the specific preventive procedures listed for your plan, as clinically appropriate and as determined by your dentist. Covered procedure codes are:
- Adult: D1110, D0120, D0274, D0140, D0220
- Perio: D4910, D0120, D0274, D0140, D0220
- Child: D1120, D0120, D0272, D1208, D0140, D0220
The number and frequency of covered visits are defined by your plan. Additional visits beyond your plan allowance are billed at your member-exclusive discount.
What Your Membership Does Not Include
Membership does not include restorative, surgical, orthodontic, cosmetic, or specialty treatment. This includes but is not limited to fillings, crowns, bridges, extractions, root canals, implants, dentures, aligners, whitening, and veneers. Treatment of this kind is billed separately at the time of service, with your member-exclusive discount applied. Membership is not prepayment for future services and does not create a credit or account balance toward treatment.
Payment, Dues, and Activation
Membership is billed as a recurring monthly subscription to the payment method you provide, or annually if you elect to prepay. A one-time activation fee applies when you enroll. Dues must be current at the time of service in order to receive member benefits. If dues lapse for more than 28 days, a reactivation fee is required to restore the membership. Dues, fees, discount amounts, and covered procedures may change; we will provide advance notice of changes to current members.
Cancellation, Refunds, and Transfers
There is no long-term contract, and you may cancel at any time by contacting our office. Cancellation stops future billing. Dues already paid are non-refundable under any circumstances, including failure to schedule or attend appointments. Membership is non-transferable to another person or to another dental office. It is solely your responsibility to schedule and keep your appointments in order to use your benefits.
Clinical and Other Disclosures
All treatment recommendations are based on individual clinical judgment. Enrollment does not guarantee any particular treatment, outcome, or result, and does not obligate the practice to provide care that is not clinically indicated. Member discounts may not be combined with any other offer, promotion, or discount. This program is governed by the laws of the State of California. If any provision is found unenforceable, the remaining provisions remain in effect.
